Job Details

Assist with monitoring Annual Wellness Visit process and data, which includes patient outreach. Participate in Clinic Coordinators meeting and projects and be liaison for population health information. Assist with educational training to all clinical staff in all clinics and disseminate information on correct documentation to improve our value-based quality measures. Assist with outreach campaigns. Provide administrative support to Behavioral Health team (Triage referrals, assist with resources, gathering records, conduct pre-evaluation interviews of family, compile and write reports.)Provide front desk support when the need is high to increase and maintain superior customer service. Address billing worklist, missing charges, and denied claims. Manage quarterly environmental rounds and fire drills. Assist with maintaining compliance for patient safety. Assist with monitoring of patient screening in all CPG clinics. Other duties as assigned.
