JOB SUMMARY

The Office Assistant is responsible for providing general administrative and clerical support to ensure smooth office operations. The Office Assistant supports the patient intake process including communicating directly with patients and families, data entry, establishing and maintaining positive relationships with customers and referral sources and responding to customer requests and concerns.

DUTIES & RESPONSIBILITIES

  • Supports patient referral and intake operations of multiple service lines.
  • Ensures compliance with all state, federal, and Joint Commission referral/intake regulatory requirements.
  • Establishes and maintains positive working relationships with current and potential referral sources.
  • Ensures seamless transition of patients to home care by providing direct oversight of patient education and preparation for home care, plan of care initiation, and coordination of care with multiple service providers.
  • Maintains an understanding of the service lines provided: Home Health, Palliative & Hospice.
  • Maintains confidentiality of staff, patients and families private health and other information.
  • Answers and appropriately directs incoming calls in a professional and friendly manner.
  • Assembles admission packets for all service lines.

JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)
  • Experience in referrals/intake in a home health or hospice environment preferred.
  • Demonstrates good communication, negotiation, and public relations skills.
  • Demonstrates autonomy, assertiveness, flexibility and cooperation in performing job responsibilities.
  • Communicates effectively with staff and patients acting in furtherance of our core values.
  • Proficient with computers and office programs.
  • Maintain privacy and confidentiality.
  • Prioritizes a team and collaborative approach.
